Intra - LASIK

Laser assisted in-situ keratomileusis with femtosecond flap creation (intra-LASIK)

In this technique the laser treatment is applied beneath a protective superficial layer of cornea – the corneal ‘flap’ (see over-page). The surface corneal epithelium is not disturbed, therefore, (except in very rare circumstances). LASIK, using a mechanical microkeratome to create the corneal flap, was first developed in the mid-1990s to eliminate or reduce some of the problems associated with the original PRK treatments (pain and soreness, haze, regression, loss of contrast/visual acuity and slow recovery). The normal default procedure now, however, combines the use of a precision femtosecond laser to create the flap together with excimer laser treatment of the refractive error. Hence there are 2 lasers involved in modern LASIK treatments. The corneal flap, which is hinged superiorly, is lifted and the excimer laser is then used to change the shape / profile of the exposed central cornea. The flap is then replaced accurately and remains firmly attached. The effect, therefore, is to remove tissue from the mid-cornea, i.e. actually within the cornea, under the protective flap. This technique has several major advantages:

  • Greater predictability and stability of refractive outcome in moderate to high amounts of myopia (from around –1.00D up to –10.00D)2
  • A similar increase in predictability for hypermetropia (+1.00D to +5.00D approx)
  • No corneal scarring
  • Rapid visual recovery (within hours in some cases)
  • Return to work/driving usually within 48 hours
  • Increased comfort - LASIK is virtually pain-free and no eye pads are needed.
  • Bilateral treatments (both eyes on the same day) are possible and are now the norm.

For these reasons most centres around the world no longer carry out PRK and have changed to Wavefront-guided Intra-LASIK
